The actions must be used on a rasterized image, if the layer you apply the action to is a smart object the action will NOT work, also, I highly recommend you to have just one layer in the layer panel when you apply the action. The image should be high dimension, don’t use on images with 100x100px dimension, the outcome will look distorted.
